Open the shortcuts app. If you do not have the app, you will need to download it from the app store. It is free.

Select the "+" icon.
Select the "Add Action" button.
Type "open app" into the search bar and select open app as it shows under "scripting."
Click on "app" next to the "Open" icon.
Search for the app you want in the search bar and select the app.
Select the "filters" icon.
Select "Add to Home Screen."
Select the icon and choose your predownloaded image from your photos.
Title the new icon the app name that it opens or whatever you want it to say.
The new icon will now automatically be added to your home screen! You can click on the icon and it will take you to whatever app you linked it to.
